SCHOOL YEAR 2008 – 2009 REGISTRATION INFORMATION
School Year 2008-2009
Registration

Dear Yokosuka Co-Op Preschool Families,

School Year 2008–2009 Registration is just around the corner! The procedure for registration is similar to that of
previous years and will be conducted during two phases. Phase One is In-House Registration. The first week of In-
House Registration is dedicated to registering returning students and their siblings. The second week of In-House
Registration is reserved for waitlisted students and their siblings, as well as alumni. Phase Two is Open House
Registration. Open House Registration is held for one week and is designed to recruit new families and students
turning three by October 31, 2008. However, any family who qualified, yet was unable to register during In-House
Registration, may also register during Open House Registration.

Highlighted below you will find answers to the most common registration questions. If you have additional

REGISTRATION DATES
IN-HOUSE REGISTRATION (PHASE ONE)
  • Returning Students/Siblings Registration will begin Monday, March 17, 2008 and will continue through Friday,
    March 21, 2008 from 0745 until 1315. (Returning students/siblings may also register during Waitlist
    Students/Siblings and Alumni Registration.)
  • Waitlist Students/Siblings and Alumni Registration will begin Monday, March 24, 2008 and will continue
    through Friday, March 28, 2008 from 0745 until 1315.
  • No Registration Packets will be accepted between March 31, 2008 and April 18, 2008.
OPEN-HOUSE REGISTRATION (PHASE TWO)
  • Open-House Registration will begin Monday, April 21, 2008 from 0745 until 1315 and will continue through
    Friday, April 25, 2008.
  • No Registration Packets will be accepted between April 28, 2008 and April 30, 2008.
ON-GOING REGISTRATION
  • On-Going Registration will resume Monday through Friday from 0800 through 1315 beginning Thursday, May
    1, 2008.

REGISTRATION REQUIREMENTS AND POLICIES
  • In-House and Open-House Registration occurs on a first-come, first-served basis. It is recommended that
    you arrive well in advance of stated Registration times. Registration will start promptly at 0745.
  • Registration Packets will be accepted in person during said Registration Dates (see above) or via postal
    mail. All Registration Packets received via postal mail will be entered into a lottery and will be selected
    randomly at the conclusion of the first day of Open-House Registration. Packets received after the first day will
    be registered at the conclusion of each registration day thereafter.
  • A Sponsor/Spouse may register his/her child and one other child (with a Power of Attorney) per line visit.
  • A Spouse does not need a Power of Attorney to register his/her own child. However, if a Spouse is signing on
    behalf of the Sponsor, a Power of Attorney is required. A Sponsor’s signature may be obtained prior to the
    start of SY 2008-2009 if a valid Power of Attorney is not currently available.
  • A Power of Attorney is required for all non-Sponsor/non-Spouse registrations.
  • Registration Packets should be completed in their entirety before arriving at Registration. Please bring all
    documents as outlined on the Registration Requirements Checklist. All required documents must be
    received by May 1, 2008.
  • If your child is placed on a CLASS LIST you are required to pay ½ month’s tuition. This payment will be
    credited towards your September 2008 Tuition Fees. All September 2008 Tuition Fees must be paid in full by
    September 5, 2008.
  • If you need to withdraw your child for SY 2008-2009, you must do so no later than Tuesday, August 19, 2008
    or forfeit ½ month’s tuition. This policy applies to all students including candidates for DoDDS' Sure Start
    Program.
  • A completed Withdrawal Form must be submitted to the Office two weeks prior to the withdrawal date.
PROGRAMS AND FEES
Children must be 3 years old by October 31, 2008 to register for SY 2008-2009. Children must be capable of using
the toilet unassisted.
  • Non-Refundable Registration Fee: $50.00.
  • New Students are required to pay a $50.00 Non-Refundable Registration Fee.
  • Returning Students are required to pay a reduced $30.00 Non-Refundable Registration Fee.
Students who were waitlisted during SY 2007-2008 and that were not placed on a CLASS LIST are not required to
pay the Registration Fee for SY 2008-2009.
Children with birthdates between November 1, 2003 and October 31, 2004 can choose:
  • 5-Day Class (Monday – Friday 0830 -1300)
Tuition Fees: $364.00. ½ month’s tuition ($182) due at Registration if child is placed on CLASS LIST.        
OR
  • 3-Day Class (Monday, Wednesday, Friday 0830 -1300)
Tuition Fees: $216.00. ½ month’s tuition ($108) due at Registration if child is placed on CLASS LIST.
Children with birthdates between November 1, 2004 and October 31, 2005:
  • 2-Day Class (Tuesday, Thursday 0830 -1300)
Tuition Fees: $167.00. ½ month’s tuition ($83.50) due at Registration if child is placed on CLASS LIST.

The Yokosuka Co-Op Preschool is a private, non-profit, non-discriminatory organization supported entirely by tuition, donations and
fundraising. In accordance with the Constitution and By-Laws, the Executive Board manages the Yokosuka Co-Op Preschool under the
jurisdiction of Commander, Fleet Activities Yokosuka.

CHILD CARE ACT OF 1989; TITLE 5 U.S.C. 301
DEPARTMENT REGULATIONS; E.O.9397; AND
OPNAVINST 1700.9 “CHILD AND YOUTH
PROGRAMS.”
PURPOSE: To provide Yokosuka Co-Op Preschool
with authorization for medical treatment in
emergency situations. Identify children and
sponsors; record required immunizations; and record
known allergies and special instructions.
ROUTINE USES: Information may be furnished to
military or civilian doctors or hospitals in the course
of obtaining medical attention for children. The
SSN is necessary so that the Yokosuka Co-Op
Preschool can identify the individual and his/her
records. Information furnished may be disclosed to
any DoD component, and upon request, to other
federal, state and local governmental agencies in
the pursuit of their official duties relating to proper
child care. Finally, the information may be
disclosed to law enforcement activities for the
purpose of litigation.
VOLUNTARY DISCLOSURE: Furnishing the
information is voluntary; however, failure to provide
the requested information could result in denial of a
child’s admission to the Yokosuka Co-Op Preschool.
The data collected on this form is maintained for
“Statistical Use Only” and is protected in
accordance with the Privacy Act (93-579), OMB
Circular A-108, and DoD Directive 5400.11.
Unauthorized disclosure of this information
constitutes a violation of the Privacy Act and may
result in a fine up to $5000. Race questions comply
with OMB Standards for Maintaining, Collecting and
Presenting Data for Race and Ethnicity, dated 30
October 1997 (62 FR 58782-58790).
